 Weston Invitational Tournament Registration-- MANDATORY

 All teams are required to send a representative to tournament registration.

ALL Teams are required to register for the tournament on Friday February 17th.

Registrations are to be held at the Weston Community Centre in Regional Park between 6-9:30pm

 

WHAT TO BRING:

In order to make the registration process as smooth as possible please have all player passes, medical releases and roster in alphabetical order.

ROSTERS - Bring the original & 4 copies (5 in total) filled out and signed by your commissioner in alphabetical order to ensure smooth check in process.

Those clubs using US Club Rosters should simply print off the US Club web site roster and bring copies.

 

MEDICAL RELEASE FORM- Every rostered player must have a notarized medical release at check in. You may use the current forms you are using for travel as long as they are notarized. (This can be found in this registration pack). Please organize in alphabetical order to insure a smooth check in process

COACHES & PLAYER PASSES - All coaches & players must have valid, laminated player passes with the necessary signatures & picture. Please place these on a ring binder and have them in alphabetical order to ensure a smooth check in process.

Emergency Contact Form; To be kept on file by tournament committee so that in the event of inclement weather or an emergency we can contact coaches and team managers at their hotel, home or cell.

FYSA Code of Ethics - A FYSA Code of ethics is required to be signed on behalf of the team by a team representative and turned in at registration. This applies to any team playing of a US Club roster.
